I really really really wonder if the problem is also the power management chip is miscalculating how much trickle to send to the battery, when in sleep state. So yeah, do not keep your Li-Ion equipped device plugged in all the time. Since then it's been sitting there without a battery, since I don't really need it as a mobile device any more. After the new battery, I kept it plugged in and running 24/7, connected to the TV, and got a swelling battery again after less than two years.

That laptop was plugged in and sitting by the TV a lot of the time, but I also used it on battery power quite a bit too. I have an '06 Macbook (non-pro) and I got a swelling battery in '09, replaced under AppleCare.

My gut instinct is that my problem had something to do with driving the large monitor, combined with keeping it plugged into the AC adapter a large percentage of the time. It is also curious that the original battery's swelling went down somewhat after I removed it from service. It is still non-flat, but not like it was (also like the original battery was) before changing my usage habits. The swelling condition of the replacement battery has subsided. I also stopped leaving the MacBook in sleep mode at night, instead doing a shutdown. The MacBook had been connected to a 24" monitor (1920x1200) and did get warm to the touch on bottom left rear (not far from the video & power ports). So I considered the situation, moved all my production files over to my Mac mini, and ceased using the MacBook as a 'stealth Mac mini'. About a month ago, it was showing signs of swelling. I replaced the battery with a 3rd party equivalent. I have a 2008 MacBook (not Pro) that had a swollen battery issue earlier this year.
